Internal Face-Lift (IFL) |

The Internal Face-Lift (IFL) is a concept of three dimensional infrastructure remodeling done to create an internal correction, to enhance the loss of bone mass due to aging in the midface, to improve a receding or small chin, and improve the jawline. Defects of the bone, soft tissue, and skin due to shrinkage of the aging face or genetic defects are remodeled and rebuilt to recreate a "youthful triangle". Components used to create the youthful triangle are Malarplasty (midface implants), Mentoplasty (prejowl or chin/jowl implant), Submental Liposuction (under the chin) or Submentoplasty (tightening of the muscle) under the chin. In some cases, additional fillers are beneficial to reduce lines on the face or lips. For patients who need additional reshaping of the lower two-thirds of the face and the neck, Dr. Gilmore performs a "Facial J-Lift" to give additional correction to the jawline. The Neck lift can also be incorporated to correct heaviness of the neck. |

Malarplasty (Midface Implant) |
Malarplasty (Midface Implants) are most often used to improve midface facial structure or to minimize acne scarring. The implants are a solid, alloplastic, silicone material, which have been contoured exactly to match the facial anatomy. They are placed in a pocket in the deep structure of the face to lift and contour, giving back definition to the midface. |

Implants are inserted through small incisions inside the mouth under the upper lip. Proper symmetry is achieved by positioning the implants into place. Because suture (stitches) material is usually of a biodegradable nature, suture removal is unnecessary. |
Minimal swelling, numbness, and some bruising may occur, especially for the first few days. Brushing of the teeth is allowed the next day. Due to natural differences of the sides of the face and swelling, the face may appear asymmetrical at first. Best results are commonly visible in a few days. Malarplasty is performed under sedation with local anesthesia. The surgery takes approximately one hour. If additional reshaping of the face is desired Malarplasty can be combined with Dr. Gilmore's Facial J-Lift to give ultimate correction.

Mentoplasty (Chin/Jowl or Prejowl Implant) |
Mentoplasty (Chin/Jowl or Prejowl Implant) used to reconstruct the chin and/or jowl area to correct a receding or small chin and improve the jawline. This procedure is done to create a better profile and correct the aging or genetically defective face. Some people desire chin augmentation and jowl reconstruction alone for a receding or weak chin or in conjunction with a Facial J-Lift. |

The implant is inserted through a small incision in the natural crease under the chin. The permanent medical grade solid, alloplastic, silicone implant is then placed adjacent to the bone under the skin and soft tissue to give a natural softness and contour. The implant itself is firm in consistency; the aim is to improve bone contours underneath the soft tissue.

Submental Liposuction (Tumescent Liposuction of Double Chin)
Submentoplasty (Tightening of the Muscle "Turkey Neck" under the Chin) |
Submental Liposuction and Submentoplasty are intended to improve the "double chin" appearance when a sagging neck is not apparent. Both procedures are done through a small incision placed in the natural crease under the chin utilizing concepts of minimal access. |
Tumescent anesthesia allows for the filling of saline (salt water-balance and local anesthesia) and then followed by liposculpting techniques (liposuction) contouring of the area under the chin and jawline is evident. |
Submentoplasty is a procedure to tighten the muscles under the chin to correct the "turkey neck". As one ages the muscles and skin lose their elasticity, creating a "cording appearance", which is lifted and tightened to create a smoother appearance. |
Both procedures are done under sedation with local anesthesia. They are most often done in conjunction with a Facial J-Lift (cheek) and Neck Lift or with a Mentoplasty (chin/jowl or prejowl) implant. The utilization of the chin/jowl or prejowl filler is significant in long term results in the "cervico-mental angle" (neck), creating an improved, youthfully, and defined jawline.
